Dose titration and programming of external infusion pump for the treatment of advanced Parkinson's disease is a time-based all-inclusive service for the purpose of providing assessment and dose optimization for a patient with advanced Parkinson's disease receiving continuous subcutaneous or enteral infusion.
When to Use
- Use K180 when performing active dose titration and programming for patients on continuous subcutaneous apomorphine or levodopa-carbidopa intestinal gel (LCIG).
- Use this code specifically for face-to-face optimization of infusion pump settings when the patient's motor fluctuations require immediate adjustment of delivery rates.
Common Pitfalls
- Billing K180 on the same day as an A185 or A183 will result in a rejection, as all assessment services are mutually exclusive with K180.
- Including non-patient-facing time, such as chart review or pump data analysis performed outside the patient's presence, will lead to audit recovery of the claim.
- Failing to document precise start and stop times in the chart is the primary cause for payment adjustment to a lower fee or total claim denial.
Billing Tips
- Ensure your documentation explicitly separates the time spent on pump programming from any other administrative tasks to justify the total units claimed.
- Since K180 is all-inclusive, do not attempt to bill a separate assessment code for the same visit, even if the patient presents with a new or unrelated neurological concern.

Assessment
The start and stop times must be recorded in the patient’s permanent medical record or the amount payable for the service will be adjusted to a lesser paying fee.
The time unit is measured as the physician time spent fully devoted to the care of the patient and excludes time spent on separately billable interventions.
Unless otherwise specified in the Schedule, the calculation of time for K180 excludes (1) time devoted to any other service or procedure for which an amount is payable in addition to the K180 and (2) non-patient-facing time such as such as time spent reviewing charts, imaging, or documentation, etc.
K180 is a time-based all-inclusive service.
The service is claimed in units.
The time unit total may include time which is consecutive or non-consecutive.
