CTO supervision includes all associated Community Treatment Order (CTO) services except those related to initiation or renewal. A single all-inclusive claim for CTO Supervision is submitted once per patient per month on an Independent Consideration basis. The form provided by the MOH for elapsed times must be completed and submitted with each claim and a copy retained on the patient's permanent medical record.

When to Use

  • Use K888 for the ongoing monthly management and supervision of a patient currently under a valid Community Treatment Order.
  • Use this code for routine clinical monitoring and coordination of care that does not involve the formal legal processes of initiating a new order or renewing an expiring one.

Common Pitfalls

  • Submitting K888 for services related to the initial assessment or legal renewal of the order, which must be billed under K887 or K889 respectively.
  • Failing to submit the mandatory MOH form for elapsed times, which results in an automatic rejection or nil payment for the claim.
  • Including travel time within the time units reported on the MOH form, as travel time is considered an insured service but is not eligible for payment.

The form provided by the MOH for elapsed times must be completed and a copy retained on the patient's permanent medical record.

For services based upon the number of 'units' of service rendered, the physician must record on the patient's permanent medical record or chart the time when the insured service started and ended. If the patient's permanent medical record or chart does not include this required information, the service is not eligible for payment.

Travel to visit an insured person within the usual geographic area of the physician's practice is a common element of insured services. Time units for any CTO services based in whole or in part on travel time are therefore insured but payable at nil.

Travel time and expenses related to appearances before the Consent and Capacity Board are not insured.
