K889CTO renewal

CTO renewal includes the completion of the CTO form and all preceding Community Treatment Order services directly related to the CTO renewal. A single all-inclusive claim for CTO Renewal is submitted once per patient per physician per renewal in any six month period on an Independent Consideration (IC) basis. The form provided by the MOH for elapsed times must be completed and submitted with each claim and a copy retained on the patient's permanent medical record. When to Use

  • Use K889 when you are formally renewing an existing Community Treatment Order, which encompasses the completion of the renewal form and all preparatory clinical assessments.
  • Select K889 instead of K888 (CTO Supervision) when the primary activity is the administrative and clinical process of re-authorizing the order rather than routine monthly monitoring.

Common Pitfalls

  • Failure to submit the mandatory MOH elapsed time form alongside the claim will result in an automatic rejection or a payment of nil.
  • Billing K889 more than once in a six-month period per patient per physician is prohibited and will trigger an audit or rejection.
  • Including travel time within your usual geographic area as part of the total time units is a billing error, as these units are insured but payable at nil.

Billing Tips

  • Calculate your total units by summing the actual elapsed minutes of all insured activities, dividing by 30, and rounding to the nearest whole unit to ensure accurate IC processing.
  • Ensure your chart notes explicitly document the start and end times for every component of the renewal process to support the elapsed time form submitted to the Ministry.

The form provided by the MOH for elapsed times must be completed and submitted with each claim.

A copy of the MOH form for elapsed times must be retained on the patient's permanent medical record.

The physician must record on the patient's permanent medical record or chart the time when the insured service started and ended.

Travel to visit an insured person within the usual geographic area of the physician's practice is a common element of insured services. Time units for any CTO services based in whole or in part on travel time are therefore insured but payable at nil.

Travel time and expenses related to appearances before the Consent and Capacity Board are not insured.
