When to Use
- Use L190 for the collection of a cervical or vaginal smear for cytology when performed as a standalone procedure.
- Bill L190 when performing a Pap test during a visit where the primary purpose is not a periodic health examination, as it is not included in the fee for a minor assessment.
Common Pitfalls
- Do not bill L190 in conjunction with a periodic health examination (K013) or a general assessment (A007), as the Pap test is considered an inclusive component of these comprehensive visits.
- Avoid billing L190 if the specimen collection is performed by a nurse or other staff member, as the physician must personally perform the procedure to claim the fee.
Billing Tips
- If a pelvic exam is performed in addition to the smear collection, ensure you are not also billing for a separate pelvic exam code unless it meets the specific criteria for a diagnostic procedure distinct from the smear collection.
