When to Use
- Use L606 for the simple excision of a benign skin lesion, such as a skin tag or small seborrheic keratosis, that does not require complex closure.
- Select L606 when performing a diagnostic biopsy of a superficial skin lesion where the primary intent is tissue sampling rather than definitive surgical removal.
Common Pitfalls
- Billing L606 in conjunction with a minor assessment code (A007) for the same lesion is a common rejection; the assessment is considered included in the procedure fee.
- Using L606 for lesions requiring excision with complex closure or significant reconstruction, which should instead be billed under the Z-series surgical codes.
Billing Tips
- Ensure the lesion site is clearly documented in the chart to support the claim if audited, as L606 is a flat-fee procedure code.
