When to Use

  • Use L722 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, when no complex reconstruction or closure is required.
  • Select L722 when performing a simple shave or snip excision of a superficial lesion that does not involve deep tissue or require a pathology report for malignancy.

Common Pitfalls

  • Billing L722 in conjunction with a minor assessment code (A007) is a common rejection; the assessment is considered included in the procedure fee.
  • Attempting to bill L722 alongside more complex excision codes like Z301 or Z302 for the same lesion will result in a rejection for duplicate billing.
  • Using L722 for lesions requiring formal excision with sutures is incorrect; these should be billed under the Z-series excision codes based on size and location.

Billing Tips

  • If multiple lesions are removed during the same encounter, apply the appropriate multiple procedure rules (usually 100% for the first and 85% for subsequent) to the L722 fee.
