Common Pitfalls
- Do not bill N127 if the procedure is part of the global surgical period for the primary craniotomy, unless the re-opening is for a distinct, non-routine complication like a haematoma.
- Avoid using N127 for initial craniotomy procedures; it is strictly for re-opening or secondary flap management, not the primary access.
Billing Tips
- Ensure the operative report clearly distinguishes the re-opening procedure from the primary surgery to justify the additional fee and avoid global period denials.
- If the re-opening is performed on an emergency basis, ensure the appropriate after-hours premium (E409 or E410) is appended to the claim to reflect the urgency.
