When to Use
- Use N131 as the primary procedure code for mechanical thrombectomy in patients with acute ischemic stroke due to large vessel occlusion in the anterior or posterior circulation.
- Apply N131 when the procedure involves the complete endovascular sequence, including diagnostic angiography, catheter navigation, and successful clot retrieval.
Common Pitfalls
- Billing for diagnostic angiography or catheter placement separately is prohibited, as these components are explicitly included in the N131 fee.
- Attempting to bill 'X' or 'J' prefix codes for concurrent services during the same operative session will result in automatic rejection.
- Submitting E987 or E988 multiple times for a single session is a common error; these add-on codes are payable only once per N131 claim regardless of the number of stents deployed.
