Intracranial stent, add-on to Endovascular thrombectomy (EVT) for acute ischemic stroke secondary to large vessel occlusion (N131)

When to Use

  • Use E987 when an intracranial stent is deployed to address residual stenosis or flow-limiting dissection during an N131 procedure.
  • Use E987 in conjunction with E988 if both an intracranial stent and an extracranial carotid stent are placed during the same N131 session.

Common Pitfalls

  • Billing E987 multiple times for multiple intracranial stents; it is strictly a flat fee payable only once per N131 session.
  • Attempting to bill diagnostic imaging or catheterization codes separately, which are already bundled into the N131 base fee.
  • Submitting E987 without an N131 claim, as it is a mandatory add-on code that cannot be billed as a standalone procedure.

The procedure must involve the deployment of an intracranial stent during an endovascular thrombectomy (EVT) session for acute ischemic stroke secondary to large vessel occlusion.

E987 and E988 are payable once per eligible fee code per EVT session regardless of the number of stents deployed.

N131 includes all components of the EVT including vascular access, catheter placement, angiography, any image guidance, mechanical thrombectomy and revascularization assessment.
