This service may be eligible for age-based premiums (see ), after-hours premiums (see ), and the trauma premium (see ) if applicable criteria are met.
When to Use
- Use N230 for the initial placement of a ventriculoperitoneal, ventriculoatrial, or lumboperitoneal shunt system.
- Use N230 for the complete revision or replacement of an existing shunt system where the entire apparatus is replaced.
Common Pitfalls
- Do not bill N230 alongside N245 (shunt revision) for the same operative session; N230 is the comprehensive code for the shunt procedure.
- Failure to document the Injury Severity Score (ISS) in the medical record will result in the rejection of the E420 trauma premium even if the procedure is trauma-related.
Billing Tips
- Ensure the claim includes the appropriate age-based premium (GP64) if the patient is under 16, as this is a surgical procedure in Part X of the Schedule.
- When performing multiple procedures during the same anesthetic, ensure N230 is identified as the major procedure to correctly trigger the primary fee payment.
