The fee for the surgeon (suffix A) is a flat fee, while fees for the surgical assistant (suffix B) and anaesthesiologist (suffix C) are calculated based on basic and time units.
When to Use
- Use N567 for the primary surgical repair of an intraspinal meningocoele where the procedure involves the closure of the dural sac and associated soft tissue reconstruction.
- Use this code when the procedure is performed as a standalone repair or in conjunction with spinal fusion procedures, as fusion is explicitly permitted alongside N567.
Common Pitfalls
- Do not bill any decompressive laminectomy or spinal decompression codes (e.g., N535, N536, N537, N538, N565, N566) on the same claim, as these are strictly restricted and will result in automatic rejection.
- Failure to document the Injury Severity Score (ISS) in the medical record will lead to the rejection of the E420 trauma premium, even if the procedure meets the clinical criteria for trauma.
Billing Tips
- Ensure that age-based premiums are applied correctly to both the surgeon and surgical assistant fees, as these are automatically calculated based on the patient's age at the time of the procedure.
- When performing this procedure after hours, select either the E409/E410 (surgeon) or E400B/E401B (assistant) premiums, but note that these cannot be combined with the E420 trauma premium on the same service.
