Performance of a Caesarean section with the concurrent procedure of tubal interruption. The surgical fee (P041A) is for the primary surgeon. Assistant services (P041B) are valued at 6 base units plus time units. Anaesthesia services (P041C) are valued at 7 base units plus time units. When to Use
- Use P041 when a Caesarean section is performed and a tubal interruption (ligation) is completed during the same operative session.
- Use this code instead of P018 if the patient has requested and receives a tubal interruption concurrently with the C-section.
- Use this code for the primary surgeon's fee; do not bill a separate fee for the tubal ligation as it is bundled into the P041 procedure.
Common Pitfalls
- Billing a separate tubal ligation code (e.g., S345) in addition to P041 is a duplicate billing error and will result in a rejection.
- Attempting to bill P041 for a C-section without a tubal interruption; use P018 for a standard C-section or P042 for a C-section with other specified complications.
- Failure to document the tubal interruption in the operative report, which is required to justify the use of the P041 code over the standard P018.
Billing Tips
- Ensure the operative report clearly details both the Caesarean section and the specific method of tubal interruption to support the P041 claim.
- If performing a multiple delivery by C-section where one or more includes a tubal interruption, ensure the primary code is P041 and subsequent deliveries are billed using the appropriate add-on codes E499 or E500.
