The Q053A is a fee designated for the management of new complex-vulnerable patients. It is only eligible for payment to physicians practicing within the following models: CCM, FHG, FHN, FHO, RNPGA, BSM, GHC, SJHC, SEAMO, and WHA.
When to Use
- Use Q053A when enrolling a new patient who meets the complex-vulnerable criteria, such as those with significant chronic disease burdens, cognitive impairment, or frailty, provided they are not already rostered to your practice.
- Apply this code when performing the initial comprehensive assessment and care planning for a patient transitioning from a non-rostered status into your FHO/FHN/FHG model who requires intensive management.

- Billing Q053A for an existing rostered patient is a common audit trigger; this code is strictly for new patients joining your practice.
