R070Pedicle flaps - Small/Intermediate

This surgical procedure is for the initial stage of creating a small or intermediate pedicle flap, with examples including a cross finger or cervical finger flap. Subsequent stages are billed using R071. An additional fee, E069, may be claimed for the preparation of a contracted recipient site. - Surgical Assistant (suffix B): The fee is based on 6 basic units plus time units. - Anaesthesia (suffix C): The fee is based on 7 basic units plus time units. The calculation of time units for both assistant and anaesthesia services follows the rules outlined in the General Preamble (see pages and ).

When to Use

  • Use R070 for the initial elevation and attachment of a small or intermediate pedicle flap, such as a cross-finger flap for fingertip reconstruction.
  • Use R070 when performing the primary stage of a staged flap procedure; subsequent division and inset stages must be billed using R071.

Common Pitfalls

  • Billing R070 for the division and inset stage of a flap, which is strictly reserved for R071.
  • Attempting to claim a hospital admission assessment or subsequent hospital visit on the same day as R070, which is disallowed unless it qualifies as the major pre-operative visit.
  • Failing to document the specific requirement for E069, which is only payable when the recipient site is contracted and requires additional preparation.

  • Always append E069 to the R070 claim if you are performing additional preparation of a contracted recipient site to ensure the flat fee is captured.
