R080 – Large pedicle flap
OHIP Radiology Code — INTEGUMENTARY SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
This surgical procedure involves the creation of a large pedicle flap, for example, a cross leg, deltopectoral, or forehead flap. This code represents the initial stage of the procedure. Subsequent stages are billed using R078. If preparation of a contracted recipient site is required, E069 may be added. As a surgical service, it includes all common and specific elements of surgical procedures as outlined in the Schedule of Benefits.
When to Use
- Use R080 for the initial creation of a large pedicle flap, such as a forehead or deltopectoral flap, where the tissue remains attached to its blood supply.
- Use R080 when performing the primary stage of a multi-stage reconstructive procedure involving significant tissue transfer.
Common Pitfalls
- Billing R080 for subsequent stages of the same flap procedure, which must be billed using R078 instead.
- Failing to include E069 when the recipient site requires specific preparation for a contracture, as R080 alone does not capture this additional work.
- Confusing R080 with R070 (free skin graft) or other minor flap codes; R080 is strictly for large pedicle flaps.
Billing Tips
- Ensure the operative report clearly describes the pedicle nature of the flap to justify the R080 fee over simpler integumentary codes.
- Always verify if the procedure qualifies for age-based premiums if the patient is under 16, as these are significant add-ons to the base R080 fee.
