R294 is a surgical procedure related to the musculoskeletal system. General rules for surgical procedures, including assistant and anaesthetist services, apply as outlined in the General Preamble. A second surgical assistant's service for R294 is only eligible for payment following authorization by a medical consultant, as R294 is not listed as an exception on page :104.

When to Use

Use R294 for the surgical excision of a benign bony outgrowth (exostosis) where the primary intent is the removal of the lesion itself.
Use R294 when the exostosis is distinct from other orthopedic procedures; do not bundle it if the removal is the sole surgical objective of the encounter.

Common Pitfalls

Billing R294 in conjunction with other major orthopedic procedures on the same site may trigger a 'duplicate' or 'included' rejection if the Ministry considers the exostosis removal an inherent part of the primary surgery.
Attempting to bill a second surgical assistant for R294 without prior written authorization from a medical consultant will result in automatic rejection, as it is not an exception on the GP90:104 list.

Billing Tips

Always apply the appropriate age-based premium for R294 if the patient is under 16 years of age, as this is a specific eligibility requirement for this code.
