Excision of an accessory navicular (scaphoid) bone. This procedure is listed in the Musculoskeletal System surgical procedures section of the OHIP Schedule of Benefits under the subheading for Foot and Ankle. The service includes the surgical procedure itself, and has associated basic units for anaesthesia (6 units) and surgical assistance (6 units). Suffixes A (surgeon), B (assistant), and C (anaesthetist) are applicable for this procedure.
When to Use
- Use R305 when performing a formal excision of a symptomatic accessory navicular bone (os naviculare) that has failed conservative management.
- Select R305 for the primary excision procedure, and append E587 if a concurrent procedure like a Kidner procedure or additional bone resection is performed through the same incision.
Common Pitfalls
- Do not bill R305 in conjunction with R306 (Excision of tarsal bone) for the same anatomical site, as this is considered unbundling.
- Avoid billing R305 as a stand-alone procedure if it is performed as part of a larger reconstructive foot surgery, as the Ministry may view this as inclusive to more comprehensive codes.
Billing Tips
- Ensure the operative report explicitly documents the excision of the accessory navicular bone to support the use of R305 over general soft tissue excision codes.
- If performing bilateral accessory navicular excisions, bill R305 with the bilateral modifier (suffix B) and apply the appropriate percentage reduction as per the General Preamble.
