R381 represents an orbital cranial osteotomy performed via an extracranial approach. As per the note in the Schedule of Benefits on page , the fee for this service includes the harvesting and grafting of bone or cartilage grafts.
When to Use
- Use R381 for extracranial orbital osteotomies performed for orbital decompression or correction of orbital dystopia.
- Select R381 when performing orbital rim advancement or repositioning that requires bone harvesting and grafting as part of the primary procedure.
Common Pitfalls
- Attempting to bill Z263, Z264, E975, Z265, E976, or Z266 for the graft harvest will result in an automatic rejection as these are explicitly included in the R381 fee.
- Submitting for a second surgical assistant without prior written justification will result in denial, as R381 is not on the pre-approved list for multiple assistants.

- Ensure the ISS score is clearly documented in the patient record if applying the E420 trauma premium to maximize the surgical fee.
