R465 – Finger-thumb arthrodesis

R465 is a surgical procedure for the arthrodesis (fusion) of a finger or thumb joint. Payment for the surgeon (A suffix) is a set fee. Payment for surgical assistance (B suffix) and anaesthesia (C suffix) is calculated based on a system of basic units plus time units. When to Use
- Use R465 for the definitive surgical fusion of a specific interphalangeal or metacarpophalangeal joint of the finger or thumb.
- Select R465 when performing arthrodesis for post-traumatic arthritis or chronic instability where joint preservation is no longer viable.
Common Pitfalls
- R465 is mutually exclusive with R285, R286, R316, R317, and R418; billing these together will trigger an automatic rejection.
- Do not bill R465 in conjunction with other soft tissue procedures on the same digit unless the additional procedure is separately identifiable and not considered an inherent part of the arthrodesis approach.
Billing Tips
- Ensure the operative report clearly specifies the joint fused, as R465 is a per-joint procedure code.
- If the procedure is performed on a trauma patient, ensure the Injury Severity Score (ISS) is documented in the chart to support the E420 trauma premium.
