R576 – Excision of fascia for Dupuytrens (planter fibromatosis), one or more rays
OHIP Radiology Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
A surgical procedure involving the excision of fascia for Dupuytren's contracture of the foot, also known as plantar fibromatosis. This service is applicable for the excision of fascia from one or more rays of the foot. This service is eligible for surgeon (suffix <suffix>A</suffix>), assistant (suffix <suffix>B</suffix>), and anaesthetist (suffix <suffix>C</suffix>) fees. Assistant and anaesthetist fees are calculated based on basic units plus time units. See to for detailed rules on assistant and anaesthesia billing.
When to Use
- Use R576 when performing a fasciectomy for plantar fibromatosis involving one or more rays of the foot.
- Select R576 instead of general excision codes when the procedure specifically targets the pathological plantar fascia characteristic of Dupuytren's-type contracture.
Common Pitfalls
- Billing R576 in conjunction with codes from the 'Skin Flaps and Grafts' category will result in an automatic rejection.
- Attempting to bill R576 for simple excision of a plantar fibroma without the formal excision of the fascia will lead to audit scrutiny for upcoding.
- Failing to recognize that R576 is inclusive of both plantar and digital components, leading to duplicate billing of secondary excision codes.
Billing Tips
- Always append E831 to R576 if the procedure requires a skin graft or revision surgery to capture the 30% fee increase.
- Ensure the operative report explicitly details the involvement of the fascia to justify the use of R576 over minor lesion excision codes.
