R620 is a surgical procedure for the disarticulation of a single metatarsal or phalanx. The service includes the surgeon's work (<suffix>A</suffix>), as well as fees for an assistant (<suffix>B</suffix>) and anaesthetist (<suffix>C</suffix>), both of which are calculated using a base unit value of 6 plus time units. For each additional metatarsal/phalanx disarticulation performed during the same operation, use E585.
When to Use
- Use R620 as the primary code for the first metatarsal or phalanx disarticulation performed during a surgical session.
- Use R620 for single-digit disarticulation procedures where the metatarsal or phalanx is removed at the joint level.
Common Pitfalls
- Billing R620 multiple times for multiple digits instead of using the primary R620 code followed by E585 for each subsequent digit.
- Failing to document the specific digit(s) disarticulated, which is required to justify the use of E585 add-on codes.
Billing Tips
- Always bill R620 as the first procedure and append E585 for every additional metatarsal or phalanx disarticulated in the same operative session.
- Ensure that after-hours premiums (E409/E410) are only applied to the R620 base fee if the procedure meets the non-elective criteria and time requirements.
