The fee for Z402 (Arteriotomy) is not payable in addition to this major cardiovascular surgery. The anaesthetic fee may be modified for procedures involving an aortic cross clamp depending on the level of application and extent of repair.
When to Use
- Use R790 for the primary repair of a traumatic laceration to a major artery, such as the femoral or brachial artery, requiring direct suture.
- Select R790 when performing an emergent vascular repair that does not qualify for more complex bypass or graft codes like R795.
Common Pitfalls
- Attempting to bill Z402 (Arteriotomy) alongside R790 will result in an automatic rejection, as the arteriotomy is considered an inclusive component of the major repair.
- Failing to apply the correct suffix (A, B, or C) for the surgical, assistant, or anaesthetic components will lead to incorrect payment calculations for non-elective after-hours procedures.
