Z402
Z402 – Arteriotomy
OHIP Psychiatric Code — CARDIOVASCULAR SURGICAL PROCEDURES · Schedule of Benefits
Z402 is a surgical procedure for making an incision into an artery. <br>Note: This service is not allowed in addition to other major cardiovascular surgery.
When to Use
- Use Z402 for an isolated arteriotomy performed for access or diagnostic purposes when no other major cardiovascular procedure is completed during the same session.
- Use this code for arterial exposure and incision required for the insertion of monitoring lines or temporary shunts when those procedures are not bundled into a primary surgical code.
Common Pitfalls
- Billing Z402 alongside major cardiovascular procedures (e.g., bypass or graft repairs) will result in automatic rejection, as it is considered an integral part of the primary surgical approach.
- Attempting to bill Z402 in conjunction with R-series vascular codes often triggers audit flags for unbundling, as the arteriotomy is typically included in the global fee of the definitive procedure.
Billing Tips
- Ensure the operative report clearly distinguishes the arteriotomy as a distinct, standalone event if billing it as a primary service to avoid the 'major cardiovascular surgery' exclusion.
