Subintimal dissection for recanalization of iliac/aorta arterial occlusive disease. This service includes catheter placement, angiography and any image guidance. The obtaining and interpretation of any images in conjunction with this procedure are not separately payable by any physician. Bilateral procedures are payable as separate services only when subintimal dissection is performed using separate bilateral incisions.
When to Use
- Use R879 for the primary subintimal dissection procedure to recanalize iliac or aortic arterial occlusions.
- Use R879 when the procedure requires catheter placement, angiography, and image guidance, as these are bundled into the fee.
Common Pitfalls
- Do not bill for image guidance or interpretation separately, as these are inclusive of the R879 fee and will result in a rejection.
- Bilateral procedures are only payable as separate services if you perform them through separate bilateral incisions; otherwise, bill as a single procedure.
- Avoid billing R879 on the same day as R860 or R861, as these are restricted and will trigger a claim rejection.
Billing Tips
- If you perform angioplasty at a site remote from the subintimal dissection site, ensure you append E815 to the claim to capture the additional fee for the remote intervention.
- Always document the specific incision sites if billing for bilateral procedures to justify the separate service payment.
