Wedge resection for ulcer, listed under Stomach, Gastrectomy. Assistant 7 basic units; anaesthesia 7 basic units. Add E708 with vagotomy, E713 after previous partial gastrectomy, and E793 (add 25%) when performed laparoscopically or laparoscopic assisted. A second assistant is payable only following authorization by a medical consultant, as S122 is not on the list of services where a second assistant is payable without authorization.
The medical record must establish that an insured service was provided, the service for which the account is submitted is the service that was rendered, and the service was medically necessary.
