Gastroduodenostomy or gastrojejunostomy is a surgical procedure that creates an anastomosis between the stomach and either the duodenum or the jejunum, respectively. This procedure can be performed in conjunction with a vagotomy (add E716), after a previous gastroenterostomy (add E711), or with a choledochotomy (add E721). It is eligible for a 25% increase to the surgeon's fee when performed laparoscopically or with laparoscopic assistance (E793).
When to Use
- Use S134 when performing a primary gastroduodenostomy or gastrojejunostomy for gastric outlet obstruction or peptic ulcer disease management.
- Select S134 when creating a bypass anastomosis in the context of palliative gastric surgery for unresectable malignancy.
Common Pitfalls
- Failure to append E793 when performing the procedure laparoscopically; this is a manual add-on and not automatically triggered by the base code.
- Attempting to bill a second surgical assistant without prior authorization, as S134 is not on the pre-approved list for multiple assistants.
Billing Tips
- Always verify if the procedure qualifies for E716 (vagotomy), E711 (previous gastroenterostomy), or E721 (choledochotomy) to ensure all concurrent surgical components are captured as flat-fee add-ons.
