E721 – Choledochotomy
OHIP Surgical Assists Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
An additional surgical fee for performing a choledochotomy. This service is payable in addition to the fee for a primary surgical procedure such as:
- `S134: Gastroduodenostomy or gastrojejunostomy
- S137: Pyloroplasty or gastroenterostomy plus vagotomy and cholecystectomy
- S184: Suture of intestine
- S287`: Cholecystectomy
When to Use
- Use E721 when performing a common bile duct exploration requiring an incision during a cholecystectomy (S287).
- Use E721 when a choledochotomy is necessary during a complex biliary reconstruction procedure such as a gastroduodenostomy (S134) or pyloroplasty (S137).
Common Pitfalls
- Billing E721 without an eligible primary procedure code (S134, S137, S184, or S287) will result in an automatic rejection.
- Attempting to bill E721 as a standalone procedure is invalid; it must be submitted on the same claim as the primary surgical procedure.
Billing Tips
- If you perform a choledochoscopy during the same session, ensure you also bill E704 to capture the additional fee, provided the primary procedure is S276, S280, S281, or S287.
