Gastrorrhaphy is the surgical repair by suture of a perforation or wound of the stomach. As per the note on page of the Schedule of Benefits, this service code is also applicable for the suture of a perforated duodenal ulcer. The fee for the surgeon (suffix A) is a flat rate. The fees for the surgical assistant (suffix B) and anaesthetist (suffix C) are calculated based on basic units plus time units. The basic units for a surgical assistant are 6 () and for an anaesthetist are 7 ().
When to Use
- Use S139 for the primary surgical repair of a perforated stomach wall or gastric wound.
- Use S139 for the surgical suture of a perforated duodenal ulcer as explicitly permitted by the Schedule of Benefits.
Common Pitfalls
- Do not bill S139 for simple gastric biopsies or non-perforation related procedures, as these require different surgical codes.
- Failure to document the specific location of the perforation (gastric vs. duodenal) can lead to audit scrutiny regarding the appropriateness of using S139.
Billing Tips
- Ensure the surgical assistant (suffix B) and anaesthetist (suffix C) correctly calculate their total fee using the mandatory base units of 6 and 7 respectively, plus the applicable time units.
