- Do not use S140 for duodenal ulcer repairs; use S139 for those procedures as explicitly noted in the Schedule of Benefits.

- Attempting to bill a second surgical assistant without prior authorization from a medical consultant, as S140 is not on the Ministry's pre-approved list for multiple assistants.
- Failing to document the Injury Severity Score (ISS) in the medical record when attempting to claim the E420 trauma premium, which leads to automatic rejection of the 50% increase.
