Haemorrhoidectomy, with or without sigmoidoscopy or repair of fissure(s) and/or sphincterotomy and/or anal dilation. Per , payment for S247 is only insured when the patient meets one of the following criteria: - failed a trial of non-surgical therapy - has Grade III or IV haemorrhoids - has haemorrhoids with substantial concomitant skin tags. The '#' prefix indicates that when performed in a hospital or Integrated Community Health Services Centre (ICHSC), the premises, equipment, supplies, and personnel are funded by the facility and cannot be charged to the patient ().
When to Use
- Use S247 for patients presenting with Grade III or IV haemorrhoids where non-surgical management has failed.
- Use S247 when haemorrhoids are accompanied by substantial skin tags requiring excision as part of the procedure.
- Use S247 instead of Z565 (Rubber band ligation) when the severity of the haemorrhoidal disease necessitates surgical excision.
Common Pitfalls
- Billing S247 without clear documentation of the failed non-surgical trial or the specific Grade III/IV classification, which triggers audit rejections.
- Attempting to bill S247 in addition to separate excision codes for skin tags; the code description explicitly includes these, making them bundled services.
- Billing S247 for minor haemorrhoids that do not meet the S28 criteria, as this is considered an ineligible service.
Billing Tips
- Ensure the operative report explicitly states the haemorrhoid grade or the failure of conservative therapy to satisfy the S28 requirements.
