When to Use
- Use S427 for the creation of a bladder flap (Boari flap) to bridge a distal ureteral defect, which inherently includes the ureteral re-implantation.
- Select S427 when the procedure is performed to manage complex ureteral strictures or injuries where primary ureteroneocystostomy is not feasible due to the length of the defect.
Common Pitfalls
- Do not bill S427 in addition to a standard ureteroneocystostomy code, as the re-implantation is already a constituent element of the Boari flap procedure.
- Failure to append E792 when the procedure is performed laparoscopically results in a 25% underpayment of the total surgical fee.
Billing Tips
- Always verify that the operative report explicitly describes the creation of the bladder flap, as simply performing a ureteral re-implantation without the flap does not qualify for S427.
