S758 – Hysterectomy with anterior and posterior vaginal repair and including enterocoele and/or vault prolapse repair

A type of hysterectomy, performed with or without removing the adnexa, which specifically includes anterior and posterior vaginal repair. The procedure also covers enterocoele and/or vault prolapse repair if performed at the same time. Payment for this service is subject to the general preamble and the notes in the surgical section. Payment for a second surgical assistant is eligible without special authorization. This service is not payable with various other female genital surgical procedures, as specified in the Schedule of Benefits. Add-on code E090 may be claimed for removal of ovaries with severe endometriosis, and E862 may be added for laparoscopic assistance (25% increase).
When to Use
- Use S758 when performing a hysterectomy that concurrently includes both anterior and posterior vaginal wall repairs.
- Use this code for cases involving a hysterectomy where an enterocoele or vault prolapse repair is performed during the same operative session.
Common Pitfalls
- Do not bill S758 in conjunction with S722, S760, S812, S813, S738, S741, S745, S747, S780, S781, or S782, as these are explicitly restricted.
- Avoid billing separate repair codes for anterior or posterior vaginal wall work, as these are considered inclusive in the S758 fee.
Billing Tips
- Apply add-on code E862 to increase the base fee by 25% if the procedure is performed laparoscopically or with laparoscopic assistance.
- Include add-on code E090 if you perform an oophorectomy for moderate or severe endometriosis during the same procedure.
