W269 is the periodic health visit listed under Non-Emergency Long-Term Care In-Patient Services in the Paediatrics (26) section of Consultations and Visits. Non-Emergency Long-Term Care In-Patient Services include chronic care hospitals, convalescent hospitals, nursing homes, homes for the aged, and designated chronic or convalescent care beds in hospitals and nursing homes or homes for the aged, other than patients in designated palliative care beds. A periodic health visit is performed on a patient, after their second birthday, who presents and reveals no apparent acute physical or mental illness. It must include an intermediate assessment, a level 2 paediatric assessment or a partial assessment focusing on age and gender appropriate history, physical examination, health screening and relevant counselling. It is limited to one per patient per 12 month period per physician; visits in excess of the limit are not insured ().
