Z224 – Club foot, etc. - manipulation and cast/strapping - with anaesthetic
OHIP Psychiatric Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
Manipulation of club foot or similar conditions, including the application of a cast or strapping, performed while the patient is under anaesthetic. This is a surgical procedure listed under the Musculoskeletal System section and requires anaesthesia with 6 base units.
When to Use
- Use Z224 when performing a formal manipulation of a club foot deformity that requires the patient to be under general anaesthetic or deep sedation.
- Use Z224 for the application of a corrective cast or strapping immediately following the surgical manipulation of a club foot under anaesthetic.
Common Pitfalls
- Billing Z224 alongside Z235 is a common error; Z235 is strictly for manipulations performed without anaesthetic and the two are mutually exclusive.
- Failing to document the use of anaesthesia will lead to audit rejection, as Z224 specifically requires the presence of anaesthetic services.
- Attempting to bill Z224 for routine cast changes that do not involve a formal surgical manipulation under anaesthetic.
Billing Tips
- Ensure the anaesthesiologist's services are billed separately using the appropriate anaesthesia base units (6 units) and applicable time units.
- If the patient is a child, remember to append the correct age-based surgical premium (e.g., AGE_PREMIUM_LT_1Y) to the Z224 fee to maximize the claim value.
