Z235
Z235 – Club foot, etc. - manipulation and cast/strapping - without anaesthetic
OHIP Psychiatric Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
A surgical procedure involving the manipulation of the foot and/or ankle, followed by the application of a cast or strapping, typically for the treatment of conditions such as club foot. This service is specifically performed without the use of an anaesthetic. For the equivalent procedure performed under anaesthesia, see Z224. If a plaster cast is applied outside of a hospital setting, the add-on code E584 may be claimed.
When to Use
- Use Z235 for serial manipulation and casting of congenital talipes equinovarus in an office setting without sedation or anaesthesia.
- Use Z235 for the manual correction and immobilization of pediatric foot deformities where the procedure is performed entirely without anaesthetic agents.
Common Pitfalls
- Billing Z235 when the procedure is performed under anaesthesia; Z224 must be used instead for any procedure involving anaesthetic.
- Claiming E584 for procedures performed within a hospital facility, as E584 is strictly restricted to office-based applications of plaster casts.
- Failing to apply the appropriate age-based premium, which is mandatory for patients under 16 years of age.
Billing Tips
- Always append the correct age-based premium (ranging from 10% to 30%) to the Z235 base fee to ensure accurate reimbursement for pediatric patients.
- If performing the casting in your office, ensure E584 is submitted on the same claim as Z235 to capture the additional fee for the casting materials and application.
