Adjustment of a circumferential external fixation device when performed without the use of general anaesthesia. This service is classified as a surgical procedure under the Musculoskeletal System section. For the same procedure performed with general anaesthesia, see Z281.
When to Use
- Use Z280 for the routine tightening or adjustment of pins and struts on a circumferential external fixator when performed in the clinic or emergency setting without sedation or general anaesthesia.
- Use Z280 when performing a minor mechanical adjustment to the frame geometry to accommodate patient healing or alignment, provided no general anaesthetic is administered.
Common Pitfalls
- Do not bill Z280 if general anaesthesia is used; Z281 must be used instead for those cases.
- Avoid billing Z280 alongside a consultation or assessment code for the same encounter, as the surgical procedure fee is intended to be comprehensive for the visit.
- Ensure the procedure is not miscoded as a simple dressing change or pin site care, which are not separately billable surgical procedures.
Billing Tips
- If the adjustment occurs after hours or on weekends, ensure you append the appropriate E409 or E410 premium to the Z280 claim to maximize the procedural fee.
- If the patient meets the specific Injury Severity Score (ISS) criteria for trauma, append the E420 trauma premium to the Z280 claim, noting that this premium cannot be combined with after-hours premiums.
