Z281 – Adjustment of circumferential external fixation - with general anaesthetic
OHIP Psychiatric Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
Adjustment of a circumferential external fixation device while the patient is under general anaesthetic. This service is a surgical procedure and as such, the surgeon's fee (suffix A) includes all associated pre-operative and post-operative care. The anaesthesiologist's fee (suffix C) is calculated based on 6 base units plus time units and includes the pre-anaesthetic evaluation, the anaesthetic procedure itself, and post-anaesthetic follow-up care as detailed in the of the Schedule of Benefits.
When to Use
- Use Z281 when a circumferential external fixation device requires mechanical adjustment or tightening that necessitates the administration of a general anaesthetic for patient comfort or safety.
- Select Z281 for patients who are unable to tolerate the procedure in an office or clinic setting due to pain or anxiety, provided the procedure is performed in an operating room under general anaesthesia.
Common Pitfalls
- Billing Z281 in conjunction with Z280 is a common rejection error, as these codes are mutually exclusive; you must choose the code based on the presence of general anaesthesia.
- Attempting to bill an office visit or consultation fee on the same day as Z281 is generally disallowed, as the surgical fee (suffix A) is inclusive of all pre-operative and post-operative care.
Billing Tips
- Ensure the anaesthesiologist's claim includes the appropriate base units and time units, and verify that any applicable ASA status or patient-specific add-on units (e.g., E022C, E017C) are submitted to maximize the anaesthetic claim.
