Z315 – Anterior packing - unilateral

A procedure involving the unilateral packing of the anterior nasal cavity to control hemorrhage (epistaxis). This is an Independent Operative Procedure (IOP). This service is for unilateral packing only. For control of epistaxis with a flexible endoscope, see add-on code E839.
When to Use
- Use Z315 when performing a unilateral anterior nasal pack to manage active epistaxis that has failed conservative measures.
- Use Z315 instead of Z314 (bilateral anterior packing) when only one nostril requires packing to achieve hemostasis.
Common Pitfalls
- Billing Z315 alongside a consultation or assessment code on the same day will trigger a rejection as it is an Independent Operative Procedure (IOP).
- Attempting to bill Z315 twice for bilateral packing is incorrect; Z314 must be used for bilateral procedures to avoid audit flags for duplicate billing.
- Failing to apply the 85% rule when performing Z315 alongside another surgical procedure under the same anesthesia will lead to overpayment recovery.
Billing Tips
- If you utilize a flexible endoscope to assist in the placement of the pack, ensure you append E839 to Z315 to receive the appropriate add-on payment.
- Remember that Z315 is an IOP, meaning the pre-operative assessment is generally considered included in the procedure fee unless specific criteria for a minor or partial assessment are met.
