Mediastinoscopy is an endoscopic surgical procedure used to examine the chest wall and mediastinum. As a procedure with a 'Z' prefix, it is an Independent Operative Procedure (IOP). According to the rules on page of the Schedule, if the major pre-operative visit was rendered by the same physician within the 12 months prior to the Z329, only a minor assessment (if rendered by a General and Family Physician) or a partial assessment (if rendered by a specialist) is eligible for payment on the same day prior to the IOP service. When the major preoperative assessment is rendered on the same day as the IOP, no other consultation or assessment is eligible for payment if rendered prior to the IOP service by the same physician on the same day. This service includes all common and specific elements for a surgical procedure as outlined in the General Preamble (-) and the Surgical Preamble ().
When to Use
- Use Z329 for a diagnostic mediastinoscopy performed to biopsy mediastinal lymph nodes or masses when no bronchoscopy is performed.
- Use Z329 when performing a surgical exploration of the mediastinum via endoscopic approach for staging purposes in lung cancer patients.
Common Pitfalls
- Billing Z329 alongside a bronchoscopy will trigger a rejection; use Z330 if both procedures are performed during the same operative session.
- Attempting to bill a consultation or assessment fee on the same day as Z329 is a common audit trigger; if the major pre-operative assessment occurs on the same day, no other assessment fee is payable by the same physician.
- Failing to account for the 12-month rule regarding pre-operative visits; if you performed the major pre-operative assessment within the last year, you are restricted to a minor or partial assessment fee on the day of the procedure.
Billing Tips
- Ensure the procedure is coded as an Independent Operative Procedure (IOP) and verify that no other surgical procedures performed under the same anesthetic are billed at 100% if they are secondary to the mediastinoscopy.
- Always apply the appropriate age-based premium (e.g., under 16 years) to the Z329 fee if the patient meets the specific age criteria, as this is a surgical procedure.
