Z537 – Biopsy - requiring general anaesthetic
OHIP Psychiatric Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
A biopsy of the oral cavity or pharynx that requires a general anaesthetic. This service is a variant of Z501 (Biopsy). The scope includes the nasopharynx, oropharynx, and hypopharynx unless specified otherwise. The '#' prefix indicates that for services performed in a hospital or Integrated Community Health Services Centre (ICHSC), the cost of premises, equipment, supplies, and personnel is covered by the facility's funding and cannot be charged to the patient.
When to Use
- Use Z537 when performing a biopsy of the nasopharynx, oropharynx, or hypopharynx specifically requiring general anaesthesia, as opposed to Z501 which is used for biopsies performed under local anaesthesia.
- Use this code for surgical biopsies in a hospital or ICHSC setting where the facility provides the equipment and personnel, as indicated by the '#' prefix.
Common Pitfalls
- Billing Z537 when the procedure was performed under local anaesthesia or sedation; this is a common audit trigger that will result in a downgrade to Z501.
- Attempting to bill an assistant fee (M400B) without prior authorization from a medical consultant, as assistant fees for this procedure are not automatically payable.
Billing Tips
- Ensure the anaesthesia record clearly documents the requirement for general anaesthesia to support the use of Z537 over the lower-valued Z501.
- If an assistant is medically necessary, submit the claim with M400B and include a detailed letter from the surgeon to the Ministry to secure the required authorization for payment.
