Z734 is payable for a double set up examination performed for one of two indications: 1. To rule out placenta previa. 2. For a trial of forceps that fails, which is then followed by a caesarean section performed by the same physician.
When to Use
- Use Z734 when you perform a formal double set-up examination in the operating room to definitively rule out placenta previa in a patient with antepartum hemorrhage.
- Use Z734 when a trial of forceps is attempted in the operating room and subsequently fails, necessitating an immediate transition to a caesarean section performed by the same physician.
Common Pitfalls
- Do not bill Z734 for a routine vaginal examination or a standard assessment of labor progress; it is strictly reserved for the specific high-risk scenarios defined in the Schedule.
- Billing Z734 in addition to the caesarean section fee is only appropriate when the criteria for a failed trial of forceps are met; it cannot be used as a general 'complication' or 'difficulty' add-on for standard deliveries.
Billing Tips
- Ensure your operative report explicitly documents the 'double set-up' environment (i.e., operating room readiness for immediate surgery) to support the claim during an audit.
