This service is for the excision of an obstructive tumour or stricture of the intestine, measuring less than 2 cm, performed through a colonoscope. When to Use
- Use Z764 when performing a colonoscopic excision of a stricture or obstructive tumour that is strictly less than 2 cm in size.
- Use Z764 for the mechanical or snare excision of a symptomatic obstructive lesion where the primary intent is to restore luminal patency.
Common Pitfalls
- Billing Z764 for lesions 2 cm or larger, which must be billed under Z765 to avoid audit recovery.
- Failing to document the specific size of the lesion in the operative report, which is the primary audit trigger for distinguishing between Z764 and Z765.
- Attempting to bill a separate diagnostic colonoscopy code alongside Z764; the excision code is considered inclusive of the procedure required to reach and visualize the lesion.
