When to Use

  • Use Z764 when performing a colonoscopic excision of a stricture or obstructive tumour that is strictly less than 2 cm in size.
  • Use Z764 for the mechanical or snare excision of a symptomatic obstructive lesion where the primary intent is to restore luminal patency.

Common Pitfalls

  • Billing Z764 for lesions 2 cm or larger, which must be billed under Z765 to avoid audit recovery.
  • Failing to document the specific size of the lesion in the operative report, which is the primary audit trigger for distinguishing between Z764 and Z765.
  • Attempting to bill a separate diagnostic colonoscopy code alongside Z764; the excision code is considered inclusive of the procedure required to reach and visualize the lesion.

Billing Tips

  • Always append E687 if laser debulking is performed in conjunction with the excision to capture the additional flat fee.
  • Ensure the operative note explicitly states the lesion size in centimeters to justify the use of Z764 over Z765.
