A surgical procedure for the delayed closure of a wound. It is explicitly noted that this service is distinct from debridement of a wound intended to heal by secondary intention. This procedure may be performed following other surgeries, such as a fasciotomy for compartment syndrome (e.g., in the elbow, forearm, tibia, or fibula).
When to Use
- Use Z783 for the formal delayed primary closure of a wound that was intentionally left open following a prior surgical procedure, such as a fasciotomy.
- Use Z783 when the surgical intent is to approximate wound edges after the initial inflammatory or infectious phase has resolved, distinguishing it from routine wound care or debridement.
Common Pitfalls
- Billing Z783 for routine debridement or wound care is a common audit trigger; ensure the procedure involves actual closure (suturing/stapling) rather than just cleaning.
- Do not bill Z783 concurrently with R495 or other debridement codes for the same site, as the Schedule of Benefits explicitly differentiates secondary closure from debridement intended to heal by secondary intention.
