A follow-up colposcopy with biopsy(ies) is performed as part of the investigation of an abnormal cervical cancer screening test or a lesion of the lower genital tract. This service includes the collection of cervical cancer screening specimen(s) for cytology and/or endocervical curetting if performed. It also includes the collection of specimen(s) for HPV testing if performed for cervical cancer screening related indications. As per the Schedule, a screening colposcopy is included in the assessment fee and is not billed separately using this code.
When to Use
- Use Z787 when performing a colposcopy with biopsy for a patient returning for investigation of a previously identified abnormal cervical screening result.
- Use this code when you perform an endocervical curettage (ECC) or collect cytology/HPV specimens during the same session as the biopsy, as these are bundled into the fee.
Common Pitfalls
- Do not bill Z787 alongside G394 or G365, as these assessment codes are restricted when the colposcopy service is provided.
- Avoid billing Z787 for a routine screening colposcopy, as the Schedule explicitly states that screening colposcopy is included in the base assessment fee and is not separately billable.
- Do not attempt to bill M400B as an add-on to Z787, as it is specifically excluded from eligibility for this procedure.
