A330 – Radiology Second Opinion of CT Study

A radiology second opinion of CT study is the service rendered when CT images made and interpreted by a radiologist at one institution or facility are referred to a radiologist (“consultant radiologist”) at a different institution or facility for his/her written interpretation. For the purposes of these services, “study” means all images related to one anatomical region, as these regions are listed in the payment rules below.
When to Use
- Bill A330 when a radiologist at your facility provides a written interpretation of CT images generated and interpreted at a separate institution, based on a referral.
- Use A330 for a second opinion on a CT study of the head, neck, thorax, abdomen, pelvis, extremities, or spine, provided the images originate from a different facility.
- A330 is appropriate when a referring physician requests a second opinion on a CT scan performed and initially read elsewhere.
Common Pitfalls
- Claims for A330 will be rejected if the CT images are being used for comparison with new images acquired at your facility; use A332 for MRI comparison scenarios.
- Ensure both the referring physician's written request and your written second opinion report are retained in the patient's chart to avoid audit issues.
- Do not bill A330 if the CT images were performed at your own institution, even if a second opinion is requested; this code requires images from a different facility.
Billing Tips
- A330 is limited to one claim per study per patient within a 30-day period, so confirm no prior second opinion has been billed for the same CT study.
- A330 is not billable via virtual care (video or telephone) as it is not listed in Appendix J.

Both the written request from the referring physician and the consultant radiologist’s second opinion report must be included in the patient’s permanent medical record.
Anatomical regions for CT studies are: head, neck, thorax, abdomen, pelvis, extremities (one or more), and spine (one or more segments).
A330 is not eligible for virtual care (video or telephone) as it is not listed in Appendix J.
