A770Extended comprehensive geriatric consultation

An extended comprehensive geriatric consultation is a consultation performed by a physician with a certificate of special competence in geriatrics on a patient: a. at least 65 years of age; or b. when the consultation is for the assessment of dementia; and where the physician spends a minimum of 90 minutes in direct contact with the patient, exclusive of time spent rendering any other separately billable intervention to the patient.

When to Use

  • Bill A770 when a geriatrician spends a minimum of 90 minutes in direct patient contact for a comprehensive assessment of a patient aged 65 or older, excluding any separately billable procedures.
  • Utilize A770 for a patient of any age undergoing a comprehensive assessment specifically for dementia, provided the physician spends at least 90 minutes in direct patient contact.
  • Choose A770 over A775 when the geriatric consultation requires an extended duration of 90 minutes or more of direct patient interaction.

Common Pitfalls

  • Billing A770 when the direct patient contact time is less than 90 minutes, which could lead to claim rejection or require downgrading to A775.
  • Including time spent reviewing charts, imaging, or preparing reports in the 90-minute direct contact calculation, as this non-patient-facing time is excluded.
  • Failing to document the start and stop times of the 90-minute direct patient contact in the patient's medical record, which is a mandatory documentation requirement.

  • Ensure the patient has a referral from a physician, nurse practitioner, or dental surgeon, as this is a prerequisite for billing A770.

A770 is eligible for payment when the consultation is for the assessment of dementia regardless of the patient's age.

Calculation of time excludes non-patient-facing time such as time spent reviewing charts, imaging, or documentation.

If general consultation requirements (written request/report) are not met, the fee may be reduced to a lesser assessment fee.
