A814 – Midwife or Aboriginal Midwife-Requested Assessment (MAMRA) by Video

An assessment of a mother or newborn provided by a physician upon the written request of a midwife or Aboriginal Midwife because of the complex, obscure or serious nature of the patient’s problem.
When to Use
- Bill A814 when a midwife requests a video assessment for a mother experiencing a non-emergent but complex postpartum issue, such as persistent mastitis not responding to initial treatment.
- Use A814 for a video assessment of a newborn with a mild, undiagnosed rash that the midwife feels requires physician opinion due to its unusual appearance, but does not warrant an in-person emergency visit.
- A814 is appropriate when a midwife requests a video consultation to discuss management options for a mother with anxiety during pregnancy that is becoming difficult to manage.
Common Pitfalls
- Billing A814 when the assessment is for an urgent newborn issue or exceeds 50 minutes; in these cases, A817 should be considered if criteria are met.
- Failing to obtain and retain the written request from the midwife, which is mandatory documentation for A814 claims.
- Submitting A814 without the K300A modality indicator, as this code is exclusively for video services.
Billing Tips
- Ensure the written request from the midwife specifies the complex, obscure, or serious nature of the problem necessitating the physician's assessment.
- If the assessment is urgent and the written request is delayed, document the verbal request and follow up with the written documentation as required.

Assessment
Written request from a midwife or Aboriginal Midwife must be retained in the patient record.
Urgent requests may be verbal but must be followed by a written request.
Findings, opinions, and recommendations must be submitted verbally and in writing to the midwife.
Findings, opinions, and recommendations must be submitted in writing to the patient's primary care physician or nurse practitioner, if applicable.
Designated as a 'Video Only' service in Appendix J.
Calculation of time for related special assessments (A817) excludes non-patient-facing time and other separately billable services.
