Midwife or Aboriginal Midwife-Requested Anaesthesia Assessment (MAMRAA) by Video is an assessment of a mother or newborn provided by an anaesthesiologist upon the written request of a midwife or Aboriginal Midwife because of the complex, obscure or serious nature of the patient’s problem and is payable to an anaesthesiologist for such an assessment in any setting.
When to Use
- Bill A818 when an anaesthesiologist provides a video assessment for a mother with a complex pregnancy complication (e.g., suspected fetal distress requiring potential anaesthetic intervention) requested in writing by a midwife.
- Use A818 for a video assessment of a newborn with an obscure condition (e.g., rare congenital anomaly) that requires anaesthesiologist input, as documented by a written request from an Aboriginal Midwife.
- A818 is appropriate for a video consultation requested by a midwife for a mother experiencing a serious anaesthetic risk during labour, where an in-person assessment (A816) is not feasible or necessary.
Common Pitfalls
- Failure to obtain and retain the written request from the midwife or Aboriginal Midwife will lead to claim rejection.
- Billing A818 when the assessment is not conducted via video, or if the request is not for a complex, obscure, or serious patient problem, will result in claim denial.
- Submitting A818 more than once per patient per anaesthesiologist per pregnancy, even if multiple requests are made, will cause subsequent claims to be rejected due to the usage limit.
Billing Tips
- Ensure the written request from the midwife clearly states the complex, obscure, or serious nature of the patient's problem necessitating the anaesthesia assessment.
- Document both the verbal and written reporting of findings, opinions, and recommendations to the midwife, as well as the written report to the primary care physician/NP, in the patient's chart.
