A816 – Midwife or Aboriginal Midwife-Requested Anaesthesia Assessment (MAMRAA)

An assessment of a mother or newborn provided by an anaesthesiologist upon the written request of a midwife or Aboriginal Midwife because of the complex, obscure or serious nature of the patient’s problem and is payable to an anaesthesiologist for such an assessment in any setting.
When to Use
- Bill A816 when an anaesthesiologist performs a pre-delivery assessment for a parturient with a known difficult airway, requested in writing by a midwife.
- Use A816 for an anaesthesiologist's assessment of a newborn with suspected congenital airway anomaly, upon written request from a midwife.
- Bill A816 for an anaesthesiologist's assessment of a mother with severe pre-eclampsia and a history of anaesthetic complications, requested in writing by a midwife.
Common Pitfalls
- Claims for A816 will be rejected if submitted without a copy of the written request from the midwife, unless it was an urgent verbal request that was subsequently documented.
- Do not bill A816 if the anaesthesia assessment is for a routine delivery without specific concerns raised by the midwife; this is typically covered by other anaesthesia services.
- Submitting A816 for an anaesthesiologist's assessment of the mother or newborn on the same day as the anaesthesia procedure itself will lead to rejection, as the assessment is generally included in the procedure fee (see GP92).

A written request from a midwife or Aboriginal Midwife is required.
Urgent or emergency requests may be initiated verbally but must subsequently be requested in writing.
The written request must be retained on the patient’s permanent medical record.
The assessment must include the common and specific elements of a general or specific assessment.
The physician must submit findings, opinions, and recommendations verbally to the midwife or Aboriginal Midwife.
The physician must submit findings, opinions, and recommendations in writing to both the midwife/Aboriginal Midwife and the patient’s primary care physician or nurse practitioner, if applicable.
Payable in any setting (office, hospital, etc.).
