Medical management of early or ectopic pregnancy - follow-up visit is for a visit that is a follow-up of A920 (Medical management of early pregnancy failure) or A922 (Medical management of ectopic pregnancy), whether rendered by the same physician who rendered the A920 or A922 service or by another physician.
When to Use
- Bill A921 for a follow-up appointment to monitor a patient's response to medical management of an early pregnancy failure initiated with A920.
- Use A921 when a patient returns for assessment after receiving initial medical management for an ectopic pregnancy under code A922.
- A921 is appropriate for a physician who is not the original provider of A920 or A922, but is managing the follow-up care for early or ectopic pregnancy medical management.
Common Pitfalls
- Billing A921 as the initial service instead of A920 or A922 will lead to rejection.
- Submitting A921 more than twice per pregnancy, even if medically necessary, will result in payment adjustment.
- Claiming A921 on the same day as another assessment, consultation, or counselling service by the same physician will be denied.

- Ensure the patient's chart clearly indicates the prior service was A920 or A922 to support A921 billing.
